By signing that waiver, a parent or legal guardian may authorize Carolina Soccer Factory to use a participant’s name, image, likeness, photograph, or recording for program administration, promotional, advertising, website, app, and social media purposes, as described in the waiver.

Requests concerning photographs or videos may be sent to info@carolinasoccerfactory.com. Removal may not be possible from materials that have already been printed, distributed, archived, or reposted by third parties.

6. Health and Medical Information

We may collect medical, allergy, injury, medication, emergency contact, and accessibility information provided for participant safety and program administration.

We may disclose this information to coaches, staff, emergency personnel, medical providers, insurers, or other persons who reasonably need it for safety, emergency response, or legal purposes.

Carolina Soccer Factory is not a medical provider and does not provide medical advice.

7. Marketing, Text Messages, and Push Notifications
